Submitting the application and taking part in the exam is free of charge, but you have to be an ESHRE member. You can find more info on ESHRE membership here.
The logbook should state how many treatment cycles the applicant has performed (including the start and end dates of performance of each of the methods listed in the requirements). The authenticity of the logbook must be guaranteed by a laboratory director, supervisor or clinical head on the submission statement form. This form will appear automatically when the application is submitted, and has to be sent in as a hardcopy to the ESHRE office.
The electronic application form must be submitted at the latest December 15th, 2012. The submission statement form must have reached the ESHRE office at the latest on December 23rd, 2012.
