The group responsible is the only person who can register delegates as part of a group. The contact person who made the online reservation of the group is the group responsible and the only contact person for all correspondence during the preparatory phase. The onsite responsible can be the same person, a colleague or a travel agency and should be known by ESHRE before 27 June 2012.
In case of a group pickup or hotel delivery, ESHRE needs to be informed with the contact details of the onsite group responsible, what means that this person can change the group registration during the Annual Meeting at the onsite registration desk. The group responsible is informed by means of the Pickup Form. If no name and/or mobile number is known on 27 June, no onsite change of the group registration is possible. (In that case new registrations should be made at the self registration corner.) These contact details are also the most important condition for a proper conduct of a group pickup or hotel dilvery of the workshop kit and badge of the delegates.
As soon as the onsite group responsible has picked up the congress kit, ESHRE is no longer responsible for that material.

Each person who wants to collect the badge and material of other participants has to sign a delivery form and mention the mobile phone (used during the Annual Meeting). So I can contact the onsite Group Responsible in case the participant does not find his responsible.
You will receive a registration number, I advise you to use it in all correspondence, with payments and in the subject of your e-mails. IST-xxxx
Member reduction fees only apply to members of ESHRE. Please note that although non-members can apply for membership before registering for the meeting, processing membership usually takes up to one week. So in case you plan to send in the Group Registration Form on 27 May, you must apply for membership for these participants on sending in the Group Registration Form (preferably by 15 May)!
To apply for membership for a few people, the group responsible can apply online. In case a group of people applies for membership, request a Group Membership Application form at membership@eshre.eu.
Be advised that Membership Application should be PAID BEFORE you REGISTER your delegates! Name changes of memberships cannot be processed. Membership applications that are not paid before sending in the Group Registration Form (at the latest 27 May 2012), will not give a reduced fee for the Annual Meeting.
Registrations for students and paramedical personnel must be accompanied respectively by a copy of the student card or a letter from the Head of the Department to prove their status. The reduced registration fee is only applied after the recipient of the proof of status together with the Group Registration Form at the latest on 27 May.
Student applies to undergraduate, graduate and medical students, residents and post-doctoral research trainees.
Paramedical applies to support personnel such as nurses/midwives, laboratory technicians, counselors/psychologists/social workers, ESHRE certified clinical embryologists (Bsc level). Embryologists have to register at the ordinary membership fee unless we receive a letter in which their Head of Department confirms they work in a routine environment as a lab technician.
