Companies or institutes are invited to submit group registrations, provided they have a minimum of 5 people to register. Companies must submit personal address details for each registered person. Registrations with the company address will be refused unless it is specified that the company employs the participant. All group registrations should be submitted with one single list that should include the name and address details of all participants and the activities for which they have to be registered (e.g. main programme + opening ceremony). Full invoice address, contact person (with direct phone, fax and e-mail address) must also be submitted. Group registrations will only be accepted when submitted by means of an online reservation: in this phase no names are needed. After payment the first pro forma according to the online reservation, the official Group Registration Form and Pick up Form will be sent to the group responsible. The next step is to submit the names and personal or professional address details of the participants. To register a group a group responsible should be aware of a number of conditions. To promote the cooperation it is advised to read the procedures and deadlines.
In order to secure group registrations at the early (low) registration fees, it suffices to make an online reservation of the expected number of participants (without names or address details) that may be registered for the main programme and/or any pre-congress course before the early fee deadline of 30 April 2012. This number of pre-booked participants will then be blocked until further details (i.e. name and address details of the participants) are supplied after the early deadline (30 April 2012), but NOT LATER THAN 10 JUNE 2012. After the 30 April 2012 deadline, late registration fees will apply to delegates that are registered (for the main programme and/or pre-congress courses) in addition to the pre-booked number of participants. After the 10 June 2012 deadline, on-site registration fees will apply to delegates that are registered (for the main programme and/or pre-congress courses) in addition to the pre-booked number of participants. It is advisable not to overestimate the number of pre-booked participants, since early non-member registration fees will apply to any pre-booked registration that is not accounted for by 10 June 2012.
50% of the fee for all reservations (non-member fee) will have to be paid before the early fee deadline of 30 April 2012 to ensure registration at an early fee. Member registration fees will become applicable after processing the Group Registration Form with contact details of the participants.After receipt of the Group Registration Form your registrations will be processed and confirmed. The confirmation will show a total that will be invoiced unless additional registrations are submitted for the existing group or other changes are made influencing the price. Invoices are payable upon receipt. Payment can be made by bank transfer, or by credit card. Cheques or bank drafts will not be accepted. For bank transfer details, please check the information on the payment request you will receive. For credit cards, write the credit card number, expiry date and card owner’s name on the payment request and have it signed by the card owner. This can then be faxed to the ESHRE Central Office for processing.
IMPORTANT NOTICE!All group registrations must be paid by 10 June 2012. Registrations without payment will not be validated or confirmed and delegates will not be admitted access to the congress. Payment for registrations and changes received after 10 June 2012 will be processed after the meeting. A final invoice will then be sent.
Rules of cancellation for individual registrations also apply to group registrations. Cancellations submitted after 30 April 2012 will be processed but will not be refunded. Before that date, an administrative fee of 100 Euro will be charged for each cancellation. Replacement registrations can be submitted without penalty but will be charged at the rate applicable at the time of submission.
