List of necessary supporting documents for application:


  1. The completed application form (online)

  2. Reproductive Medicine (RM) training syllabus and programme of the applying institution/centre. (Each Trainee’s weekly timetable/schedule – time frame/allocated time for each training component including protected time for personal study/research/audit)

  3. One sample of fellow log book with details for fellow in training, one logbook per each year of 3 years training per trainee

  4. Floor plan of center (in case of virtual visit: department/centre facilities video)

  5. Fellow questionnaire fulfilled (one for each fellow in training)

  6. PowerPoint presentation about centre (centre activities, number of procedures, staff structure, daily schedule, work organization, research activities etc.) - with voice-over – Guidelines with instructions will be provided after submission of online application (in case of virtual visit: to be sent in advance)

  7. PowerPoint presentation about education and RM training programme - structure, time frame and locations (again with voice-over) – Guidelines with instructions will be provided after submission of online application (in case of virtual visit: to be sent in advance)
