Webinar 1: Troubleshooting: Ovarian stimulation

This webinar will cover several problems arising during ovarian stimulation and more importantly how to troubleshoot. Clinical scenarios that will be discussed include high basal LH values in spite of pre-treatment with the oral contraceptive pill, dropping estradiol (E2) levels during ovarian stimulation, the presence of luteinized unruptured follicles (LUF) in a modified natural cycle and the lack of an endogenous LH trigger when a GnRHa trigger is used in women previously treated with the oral contraceptive pill. Additionally, the experts will discuss the routine evaluation of serum LH and/or progesterone levels prior to the trigger.

Webinar 2: Troubleshooting: The oocyte pick-up procedure

Transvaginal oocyte pick-up (OPU) is a widely performed procedure, with a low complication rate. Still, issues may arise before or during the procedure that require actions to ensure the safety of the patient and, of possible the efficacious retrieval of the oocytes.
This webinar on the oocyte pick-up procedure will discuss some clinical scenarios in OPU that require troubleshooting.

Such scenarios include:

- The woman has a SARS-CoV-2 infection
- No oocytes are retrieved (Empty follicle syndrome)
- The suction pump fails during the OPU procedure
- A small endometriotic cyst is accidentally aspirated

Additionally, the experts will discuss the indications for antibiotic prophylaxis shortly before or during OPU.

A critical step in ART is to use appropriate ovarian stimulation protocols to develop oocytes that are mature and competent. Still, a proportion of oocytes retrieved during oocyte pick-up (OPU) are found to be immature and/or abnormal.
In this webinar, panellists will discusses causes and implications of immature and/or abnormal oocytes, including “dark oocytes”, but they will also elaborate on what can be done, and how the next cycle can be approached.

Fertilisation failure is an uncommon but devastating event in ART. Fertilisation failure can provide a marker of a problem in gamete quality (sperm function, oocyte activation, gamete receptors), sperm processing, or in the number of spermatozoa used for insemination. In this webinar, the panel will explore troubleshooting upon total fertilisation failure and will explore the relevance of rescue ICSI and the practical aspects rescue ICSI, such as the timing of the fertilization check. The panel will also discuss ICSI, calcium ionophore oocyte activation and other treatment options and their relevance in preventing fertilisation failure in a next cycle.
